New Patient Registration

The practice is open to new patients.It is necessary for all new patients who wish to register with the Practice to make an appointment for a New Patient Medical.  When you register you will also be asked to fill out a medical questionnaire. This is because on occasion it can take a some time for us to receive your medical records. 

Preference of Practitioner 

Patients have the right to receive services from a particular Doctor – either generally or in relation to any particular conditions. This will be noted in your record.
